Endoscopic retrieval of a migrated stent after endoscopic ultrasound-guided choledochoduodenostomy.

چکیده
choledochoduodenostomy (EUS-CDS) is reported with increasing frequency [1]. While stent migration into the bile duct is a possible complication of EUS-CDS, it has not previously been reported. We describe a case of complete stent migration into the bile duct, where the stent was successfully retrieved by inserting an endoscope directly into the bile duct after balloon dilation of the duodenal fistula. A 73-year-old woman presented to our institutionwith recurrent cholangitis due to a congenital choledochal cyst. She underwent percutaneous transhepatic biliary drainage (PTBD) after failed biliary cannulation during endoscopic retrograde cholangiopancreatography. However, she could no longer tolerate the PTBD tube. Thus EUS-CDS was performed via the duodenal bulb followed by placement of two double-pigtail stents. After 1 month, the woman developed acute cholangitis. Urgent endoscopy revealed that one of the biliary stents hadmigrated completely into the bile duct. The migrated stent could not be grasped under fluoroscopic guidance. A guidewire was passed through the choledochoduodenal fistula along the remaining stent, and the fistula was dilated with a 15-mm dilation balloon (CRE; Boston-Scientific, Natick, Massachusetts, USA; ●" Fig. 1). Then an endoscope was directly advanced into the bile duct through the dilated fistula. Direct cholangioscopy revealed that the two double-pigtail stents were entwined with each other (●" Fig. 2). Under endoscopic guidance, the stents were disentangled using a forceps. Then the distal end of the migrated stent was grasped with a snare (●" Fig. 3) and pulled from the bile duct to the duodenal bulb (●" Fig. 4,●" Video 1). No procedure-related complications occurred, and the patient was discharged after the PTBD tube had been removed. The management of stent migration after EUS-CDS is technically more challenging than endoscopic removal of a migrated stent via the papilla [2–4]. Transmural direct cholangioscopy through the choledochoduodenostomy fistula was effective for retrieving the migrated stent. Endoscopic ultrasound-guided pancreaticogastrostomy for symptomatic pancreatic duct obstruction caused by migrated pancreatic stent
Endoscopic pancreatic stenting has been widely used in various pancreatic conditions. With the increasing use of pancreatic stents, many complications have been recognized. Especially, proximal stent migration presents a serious condition because of subsequent pancreatic duct obstruction, impaired drainage, ductal dilation, and pancreatic pain. Although endoscopic retrieval is the preferred tre...
متن کاملDifficult Endoscopic Retrieval of a Migrated Stent inside a Pseudocyst
Endoscopic ultrasound (EUS) guided pseudocyst drainage can have complications such as bleeding, perforation, infection, and stent migration. We report a case of iatrogenic migration of stent inside a large dumbbell-shaped pseudocyst managed successfully with a difficult endoscopic retrieval procedure.
متن کاملWire-Guided Endoscopic Snare Retrieval of Proximally Migrated Pancreatic Stents after Endoscopic Papillectomy for Ampullary Adenoma
With the increasing use of pancreatic duct (PD) stents after endoscopic papillectomy (EP), complications such as proximal migration of the stent have become increasingly prevalent. A PD stent that migrates within a nondilated PD may be difficult to remove endoscopically. We performed endoscopic retrieval of proximally migrated PD stents after EP in 5 patients. Endoscopic retrieval was performed...
متن کاملEndoscopic Ultrasound-Guided Biliary Drainage Using a Fully Covered Metallic Stent after Failed Endoscopic Retrograde Cholangiopancreatography
Background and Study Aims. Endoscopic ultrasound- (EUS-) guided biliary drainage (EUS-BD) is an alternative treatment for biliary obstruction after failed endoscopic retrograde cholangiopancreatography (ERCP). In this study, we present the outcomes of inpatients with obstructive jaundice treated with EUS-BD using a fully covered metallic stent after failed ERCP. Patients and Methods. A total of...

Endoscopic ultrasound (EUS)-guided hepaticogastrostomy with a fully covered metal stent is an option for malignant biliary obstruction after a failed endoscopic retrograde cholangiopancreatography (ERCP) [1–3]; however, migration of the stent can be a fatal complication [3, 4]. We report a case in which a migrated stent was successful reset using a foreign body forceps. A 73-year-old woman deve...
